The forms packet includes the following:

| Youth Registration form
Each youth attending Youth Strike for Christ must fill out
this form and be signed by parent or legal guardian
|
| Adult Registration form
Each adult attending Youth Strike for Christ must fill out
this form and be signed
|
| Medical Release form
This form must be filled out for each youth and adult
attending Youth Strike for Christ.
|
| Room Assignment form
- Youth leaders make room assignments for youth and
adults at Youth Strike for Christ.
- This year you can pick how many
people you want in your hotel room (there is a price
change based on different number of people
in your room) This form details all the
different options and cost.

Full payment must be included with all form to insure you are registered
for Youth Strike for Christ.
When mailing in forms, please make sure you mail all the following forms
together:
Youth Registration Form (One form must be filled out for each youth)
Adult Registration Form (One form must be filled out for each adult)
Medical Release Form (One form must be filled out for each youth and
adult)
Room Assignment Form (Please read instruction carefully on this
form)
(Youth leaders should mail in all forms at one time for your entire group)
One adult required for every 5 youth attending Youth Strike for Christ!

REGISTRATION DEADLINE is Jan 15, 2011
Cancellations: If you need to cancel your reservation, substitutions
of the same sex as previous registered are welcomed as long as all registration
information for the substituted individual is provided to Youth Strike for
Christ.
REFUND: Refunds are allowed up to Dec 1, 2010. There are NO refunds
after Dec 1, 2010.
The Youth Strike for Christ event will take place regardless of weather
conditions. No refunds will be given incase of bad weather.
